THE STRATEGIC PLAN FROM OUR COMMUNITIES TO REDUCE THE HIGH INCIDENCE OF DIABETES IN
As part of the STAT campaign to Prevent Diabetes, it is immediately invited to voluntarily join Mayors and community leaders of organizations of municipalities, community of faith, this is the best alternative to the health costs of population.

Comunidad Saludable de al Montaña is a recognized organization in the National Diabetes Prevention Program of the Centers for Disease Control.
BENEFITS FOR MUNICIPALITIES
The opportunity is provided free of cost of advisory and operational services to each municipality, community organization, faith-based and private sector business to join in establishing an island-wide DPP program.

Our goal for 2018 is to create 30 spaces to provide Program services Prevention of Diabetes in our communities.

​Comunidad Saludable de la Montaña aware of the health problem caused by Diabetes Mellitus and its complications in our country we present the initiative to provide part of the official Medicare and Medicaid services accessible to the people as a project made by Puerto Ricans for underserved populations throughout the island including especially small government and private industry workers business.
COMMUNITY ENVIRONMENT OF THE DPP
​Comunidad Saludable de la Montaña will provide the digital platform and operation necessary to offer services with training for coaches that will make it possible for a community location to comply with these parameters, it can be a service area.
​
What type of location can be certified?
A "community setting" means a place where the MDPP provider provides MDPP services outside of their administrative locations. A community setting is a place open to the public and not primarily associated
with the provider. Community settings may include, for example, community pharmacies, community laboratories, physicians' offices, community health centers, church halls, schools, or multipurpose rooms in recreation centers.

FORMALITIES PROGRAM ON SITE

The goal of CDC and Centers for Medicaid and Medicare Services (CMS) establish this expansion model throughout the United States and its territories.
​
The Board of Directors of ​ Healthy Mountain Community has decided its strategic plan for 2018 is carried out in community facilities to stimulate a culture of health prevention outside of hospitals and doctors' offices and to promote preventive care in healthier people.
​ Comunidad Saludable de la Montaña has all the certifications to carry out this project throughout Puerto Rico, given our initiative we have put the DPP at the service of the people from Puerto Rico.
​
It is estimated that in Puerto Rico there are over 1,000,000 million people who have prediabetes these people can develop Type 2 Diabetes from 3 to 5 years once they are diagnosed with prediabetes.
​
Federal requirements require several elements that CSDLM, being a recognized official organization, must ensure that they are met through a proprietary business model.

2) The entity is committed to creating a collaborative environment that comply with the standards and procedures of Comunidad Saludable de la Montaña recognized entity to provide services of the Diabetes Prevention Program.

3) Provide classrooms equipped with Wifi internet for group meetings of 15 people that comply with the standard and federal rules established by CMS. [ Download the suggested standard templates ]

If there are fewer cases of type 2 diabetes in the United States, that means that communities will be healthier; the workforce will be healthier and more productive, and health care costs will be lower for all.
