WHO IS ELIGIBLE?

​

To be eligible for referral to a CDC-recognized lifestyle change program, patients must:

​

1. Be at least 18 years old 

2. Being overweight (body mass index ≥25; ≥23 if Asian)

3. Not be pregnant

4. You do not have a previous diagnosis of type 1 or type 2 diabetes

5. Get a blood test result in the prediabetes range in the last year:

6. Hemoglobin A1C: 5.7–6.4% or

7. Fasting plasma glucose: 100–125 mg / dL or

8. Two-hour plasma glucose (After a 75-g glucose load): 140–199 mg / dL or

9. Have a previous clinical diagnosis of gestational diabetes or

10. Take the prediabetes risk test and receive a high-risk type 2 diabetes screening result.

WHO IS NOT ELIGIBLE?

​

1. Current diagnosis of diabetes (ICD-9: 250.xx; ICD-10: E10.x, E11.x, E13.x and O24.x) or

2. Current insulin use

INELIGIBLE PATIENTS MAY PARTICIPATE IN THE PREVENTIVE AND WELLNESS SERVICES PROGRAM
